All travelers are required to complete this electronic form Will open in a new window. no more than 48 hours prior to entering Cuba. Passengers travelling for leisure must select Tourism in the travel reason field when completing the form to obtain an electronic visa number. Once the form is completed, travelers will receive a QR code that must be presented, electronically or a printed version, to authorities upon arrival. Without the QR code, boarding the aircraft will be denied.
